Aspects électrocardiographiques et échocardiographiques de l’hypertendu âgé au CHU de Conakry.

Electrocardiographic and Echocardiographic Aspects of the Elderly hypertensive patients at the University Teaching Hospital of Conakry.

RESUME

Introduction : L’hypertension artérielle (HTA) du sujet âgé est un facteur indépendant de maladie cardio-vasculaire. De même, il s’agit d’un indicateur indépendant et puissant significativement associé à l’augmentation de la morbi-mortalité par le biais de l’infarctus du myocarde, l’insuffisance cardiaque, l’insuffisance rénale et de l’atteinte cérébro-vasculaire.

 

Méthodes : Nous avons réalisé une étude descriptive transversale sur une durée de 12 mois, allant du 1er octobre 2023 au 30 septembre 2024.

 

 Elle incluait les patients âgés de 60 ans ou plus :

 

-           ·ayant une hypertension artérielle (HTA) connue ou diagnostiquée selon les critères de l’ESC/ESH (pression artérielle ≥ 140/90 mmHg ou traitement antihypertenseur en cours) ;

 

-           ayant bénéficié à la fois d’un électrocardiogramme (ECG) de surface et d’une échocardiographie Doppler cardiaque dans le cadre de leur suivi ambulatoire ;

 

-           ayant accepté de participer après consentement éclairé.

 

Résultats : Au total, 458 patients âgés hypertendus sur un nombre total de 1920 patients reçus pendant la période d’étude, soit une fréquence de 23,8%, avec une prédominance féminine soit 67,5% (tableau 1). La tranche d’âge la plus représentée était celle comprise entre 60 et 69 ans avec une fréquence de 70,5%. HVG, hypertrophie de l’oreillette gauche, et trouble de rythme ventriculaire à type d’extrasystole ventriculaire étaient les anomalies les plus retrouvées à l’électrocardiogramme avec des fréquences respectives (60,2%), 35,6% et 22,2% (tableau 2).

 

 L’échocardiographie Doppler était effectuée chez tous nos patients. La MVG moyenne était de 107,4 ± 35,2 g/m². L’HVG était retrouvée chez 245 patients (53,4%), à prédominance concentrique 33,8% contre 19,6% d’HVG excentrique (tableau 4)

 

Conclusion : Cette étude avait pour objectif d’évaluer la prévalence et les caractéristiques de l’hypertrophie ventriculaire gauche chez le sujet âgé hypertendu, ainsi qu’à comparer sa détection par électrocardiogramme et échocardiographie Doppler au CHU de Conakry.

Les résultats obtenus mettent en évidence une forte prévalence des anomalies cardiaques structurelles et électriques, dominée notamment par l’hypertrophie du ventricule gauche (HVG) et de l’oreillette gauche. L’électrocardiogramme s’est révélé plus sensible que l’échocardiographie dans la détection de l’HVG.

SUMMARY

Introduction: high blood pressure (HBP) in elderly patients is an independent factor of cardiovascular disease. Similarly, it is an independent and powerful indicator significantly associated with increased morbidity and mortality through myocardial infarction, heart failure, renal failure and cerebrovascular involvement.

 

Methods: we conducted a cross-sectional descriptive study over a period of 12 months, from October 1, 2023 to September 30, 2024. It included patients aged 60 or over, with known or diagnosed high blood pressure (HBP) according to the ESC/ESH criteria (blood pressure ≥ 140/90 mmhg or antihypertensive treatment in progress), having benefited from both a surface electrocardiogram (ECG) and a Cardiac Doppler Echocardiogram as part of their follow-up outpatient and also, having agreed to participate after the informed consent.

 

Results: in total, 458 elderly hypertensive patients out of a total number of 1920 patients seen during the study period or a frequency of 23.8%, with a female predominance of 67.5% (table 1). The most represented age group was between 60 and 69 years old with a frequency of 70.5%. Left ventricular hypertrophy (LVH), left atrial enlargement (LAE), and ventricular arrhythmias such as ventricular extrasystole were the most common abnormalities found on the ECG with respective frequencies of 60.2%, 35.6% and 22.2% (table 2). Doppler echocardiography was performed in all our patients. Left ventricular mass mean (LVM) was 107.4 ± 35.2 g/m². LVH was found in 245 patients (53.4%), predominantly concentric LVH 33.8% versus 19.6% eccentric LVH (table 4).

 

Conclusion: this study aimed to evaluate the prevalence and characteristics of left ventricular hypertrophy in the elderly hypertensive, as well as to compare its detection by ECG and Doppler echocardiography at the University Teaching Hospital of Conakry.

The results obtained highlight a high prevalence of structural and electrical cardiac abnormalities, dominated particularly by the left ventricular hypertrophy and left atrial enlargement. The ECG was found to be more sensitive than echocardiography in the detection of left ventricular hypertrophy.
