Evaluation of the 4PEPS score in the diagnosis of pulmonary embolism at the University Hospitals of Lomé (Togo).

RESUME

 

Introduction : Le diagnostic de l’embolie pulmonaire (EP) reste un défi en pratique clinique, en particulier dans les contextes à ressources limitées où l’accès aux examens spécialisés peut être restreint. Le score 4 PEPS, récemment proposé comme outil décisionnel clinique simplifié, pourrait optimiser l’orientation diagnostique des patients suspects d’EP. Cependant, son utilité n’a pas encore été évaluée dans les structures hospitalières du Togo.

 

Objectif : Évaluer la performance diagnostique du score 4PEPS dans la prise en charge de l’embolie pulmonaire (EP) aux Centres Hospitaliers Universitaires (CHU) de Lomé.

 

Méthodes : Une étude observationnelle, transversale et multicentrique à collecte rétrospective a été menée du 1er août 2024 au 31 août 2025 dans les services de cardiologie et d’urgences des CHU Campus et Sylvanus Olympio. Ont été inclus tous les patients adultes suspects d’embolie pulmonaire ayant bénéficié d’une évaluation selon le score 4PEPS et, en cas de probabilité non faible, d’un angioscanner thoracique. Les données cliniques, biologiques et radiologiques ont été analysées à l’aide du logiciel SPSS 26.0.

 

Résultats : Un total de 412 patients a été inclus (âge moyen : 57,8 ± 16,2 ans ; 67,4% de femmes). Les principaux facteurs de risque étaient l’alitement prolongé (38 %), l’obésité (27 %) et les antécédents de thrombose veineuse profonde (19 %). L’angioscanner thoracique a confirmé une EP chez 175 patients. Le 4PEPS a montré une très forte sensibilité (96 %) et une VPN de 88 %. L’analyse de la courbe ROC a montré une AUC de 0,89 [IC95 % : 0,80 – 0,91], traduisant une bonne capacité discriminante du score 4PEPS pour le diagnostic d’embolie pulmonaire. Le taux de faux négatifs était de 2,3%.

Conclusion : Le score 4PEPS constitue un outil simple, performant et adapté au contexte africain pour évaluer le risque d’embolie pulmonaire. Son utilisation systématique pourrait réduire le recours excessif à l’imagerie et améliorer la prise en charge diagnostique au Togo.

SUMMARY

 

Introduction: Diagnosing pulmonary embolism (PE) remains a challenge in clinical practice, particularly in resource-limited settings where access to specialised tests may be restricted. The 4PEPS score, recently proposed as a simplified clinical decision-making tool, could optimise the diagnostic referral of patients suspected of having PE. However, its usefulness has not yet been evaluated in hospitals in Togo.

 

Objective: To evaluate the diagnostic performance of the 4PEPS score in the management of pulmonary embolism (PE) at the University Hospital Centres (CHU) in Lomé.

 

Methods: An observational, cross-sectional, multicentre and retrospective study was conducted from 1 August 2024 to 31st August 2025 in the cardiology and emergency departments of the Campus and Sylvanus Olympio University Hospitals. All adult patients suspected of having pulmonary embolism who were assessed using the4PEPS score and, in cases of high probability, underwent a chest CT angiogram were included. Clinical, biological and radiological data were analysed using SPSS 26.0 software.

 

Results: A total of 412 patients were included (mean age: 57.8 ± 16.2 years; 67.4% women). The main risk factors were prolonged bed rest (38%), obesity (27%) and a history of deep vein thrombosis (19%). Chest CT angiography confirmed PE in 175 patients. The 4PEPS showed very high sensitivity (96%) and a NPV of 88%.ROC curve analysis showed an AUC of 0.89 [95% CI: 0.80–0.91], reflecting the good discriminatory ability of the 4PEPS score for the diagnosis of pulmonary embolism. The false negative rate was 2.3%.

Conclusion: The 4PEPS score is a simple, effective tool that is well suited to the African context for assessing the risk of pulmonary embolism. Its systematic use could reduce the excessive use of imaging and improve diagnostic management in Togo.
